CrownPointe Communities owns and operates twelve assisted living campuses in three states and a senior condominium-only campus in Muncie, Indiana. Historically, the company has concentrated its efforts in secondary markets in counties with populations of less than 50,000. However, with campuses acquired since 2002, CrownPointe now operates campuses in larger metropolitan areas as well.

Our focus is to provide a continuum of care within a campus atmosphere that will provide both independent and assisted living for our residents. Many of our campuses now provide these multi-tiered services through condominiums, affordable housing and assisted living for our senior population.

Additionally, CrownPointe Communities and its managing partner, Heritage Medical Group, develop primary care or physician practices for many of the communities through a separate home health component and a nurse practitioner program offered through independent medical corporations. These programs enable residents and families to contract for services that go beyond the service offerings normally associated with assisted living.
